Please click the link below to learn in detail how sick is too sick to come to school. Please abide by these guidelines so illnesses are not spread to other students/Summit staff. No med 'em and send 'em to get them through the school day. We usually end up having to call a parent to come get the student when the medication wears off.

How Sick is Too Sick?

Remember
- No fever, vomiting, diarrhea, or severe cough for the previous 24 hours.
- If a student shows these symptoms at school parents will be called to pick them up as soon as possible.
- No med 'em and send 'em.

Please note that for Summit staff to be able to give ANY medication to a student we must have a medication agreement on file for that medication. This includes pain relief, allergy meds, cough drops, etc. We cannot administer ANY medication (prescription or OTC) without a doctor signed medication agreement or a doctor signed care/health plan on file.

Students are not allowed to carry any medications (pain relief, allergy, cough drops, etc.) on their person. If a student is in need of medication while at school and they do not have a medication agreement or health/action plan on file, a parent may come to the school and administer the medication to the student.

If you have a student with Type 1 Diabetes, please contact our school nurse consultant, Dawn Kollmorgen (dkollmorgen@jajags.com), to establish orders and create a detailed health plan.

You have the following three options to fulfill CDPHE's requirements regarding vaccination:

1) You may submit up to date copies of your students' vaccination records.

2) You may utilize THIS medical immunization waiver if applicable. Please complete this waiver and have it signed by your physician before submitting it to Summit. This waiver does not expire and does not need to be submitted each year.

3) You may utilize a nonmedical immunization waiver, to do so please complete one of the two following options:

A) THIS nonmedical immunization waiver must be filled out by you, signed by your health care provider and then submitted to Summit. Waiver must be completed each year and dated July 1st or after.

A nonmedical waiver must be signed by:

• Advanced practice nurse (APN)

• Pharmacist (Pharm)

• Physician (Medical Doctor [MD] or Doctor of Osteopathic Medicine [DO]) 

• Physician assistant (PA) 

• Registered nurse (RN)

OR

B) You may choose to watch the Online Education Module from CDPHE. Upon completion of the module you will be directed to print an exemption certificate. Please print the certificate and submit it to Summit. We must receive a certificate for each student. You do not need to watch the video for each student, but will need to go back a screen and print a certificate for each student. This module must be completed and the exemption certificate submitted each year and must be dated after July 1st.
