Summit Waivers

• A Comprehensive Waiver must be completed for each KG-12th grade student.

• All 9th-12th graders must have the 9th-12th Grade Agreement completed by a parent/guardian.

• Any parent/guardian who will be volunteering at Summit must complete the Volunteer Waiver.

• All waivers are to be completed electronically. No paper forms will be accepted.

1) Comprehensive KG-12 Waiver
Go HERE to complete Comprehensive Waivers for your student(s).

2) 9th-12th Grade Agreement

Our 9th-12th Grade Agreement needs to be completed by a parent/guardian only. Students no longer need to sign the form, though they do need to READ THE FORM. Go HERE to complete the form for each of your 9th-12th graders.

3) Volunteer Waiver
Any person volunteering at Summit in any capacity must complete THIS waiver.
